Geriatric Care Management Services An assessment and Care Plan includes meeting with the senior citizen three times over a 10 day period to assure that all of their physical, mental, emotional, financial and environmental strengths and needs are assessed. We evaluate the ability of the elder to live alone and provide resources and/or referrals that enable the senior adult to remain in the home as long as possible. We provide a variety of options, along with a cost analysis of each one. We inform you of all available elder care resources, facilities, and programs and help you develop a care plan that best fits your needs. Geriatric Assessment

Care Plan: All geriatric care management services provided by Senior Advocates of Brevard are billed at an hourly rate, in 10-minute increments. In consultation with the geriatric care manager, the family and elderly person choose the type and frequency of services to be provided. The cost of geriatric care management services is agreed upon in advance and depends on the level of intervention requested. Billing is done on a monthly basis, unless other arrangements are made. Bill Paying and Insurance Filing services are billed at a rate of $50.00 per hour and can take several forms. We can meet face-to-face monthly with the senior client to review bills and insurances and assist in check preparation and mailing. Or, we can receive, process and pay bills directly each month. Medicaid Application Assistance Assisting with a Medicaid application is billed at a rate of $75.00 per hour. Applying for ICP (nursing home) Medicaid and Medicaid Waiver (in-home care) is a complicated process requiring the collection and dissemination of numerous documents and financial statements. Each individual’s specific situation will dictate the time required to successfully complete this process. |
1) Many of the families that hire us do so as the result of a “situation” or crisis. Initially, we may spend between 6-12 hours to stabilize a presenting crisis situation and subsequently, it will require between 2-4 hours a month to maintain it. If we're part of a Doctor's written order, our services can be tax deductible.
2) Some families may begin to have concerns that their parent is beginning to forget things, or are evasive when questioned about ongoing medical conditions. These families will hire Senior Advocates of Brevard to conduct our assessment and care plan.
Senior Advocates of Brevard will arrange, monitor, and supervise all services that are provided to our clients’ households. |
